F/T Position Summary:
Service Coordinators engage in an active, ongoing process to assist and enable Help Me Grow-Part C eligible children and families to receive the rights, procedural safeguards and services authorized to be provided under the Help Me Grow-Part C program and the federal regulations for Part C of IDEA.
Supervisory Responsibilities:
None Essential Job Functions:
Provides visits to children, 0-3 years of age, with a suspected or diagnosed developmental delay and/or disability or qualifying medical diagnosis. Conducts visits in the child/family home or other natural environment(s) where the child participates in his/her daily routines Facilitates and participates in the development, implementation, review and monitoring of the Individualized Family Service Plan for each assigned child. Facilitates positive family outcomes. Informs monitors and facilitates the timely delivery of services to the family. Coordinates with medical and health care providers. Coordinates linkages between families and additional supports and/or advocacy providers. Coordinates completion of all required eligibility determination procedures in accordance with the Part C Eligibility Determination policy. Coordinates the completion of ongoing screenings and assessments. Facilitates a transition plan to educational services by the child's third birthday. Maintains and provides accurate and timely statistical data, records, and reports. Other duties as assigned.
Additional Job Functions:
Actively participates in reflective supervision sessions with Director to identify and solve issues and challenges
Necessary Education and Training:
Professional Degree, Bachelor's Degree in Social Work, Nursing, Education or a related field
  • Professional Licensure preferred 2 years positive work experience with children and/or families Experience with low-income individuals and families encouraged Culturally competent Bilingual (English/Spanish) helpful Driver's License and use of a vehicle for daily travel within assigned areas Flexibility in order complete home visits so as to accommodate varying schedules of participating families and contribute to special events promoting the agency and/or program.
Physical Demands:
In order to perform the essential functions of this position successfully the employee may be required to sit, stand, reach, lift, bend, kneel, stoop, climb, push and pull items weighing 50 pounds or less. This position requires manual dexterity, auditory and visual skills, and the ability to follow written or oral instructions and agency procedures. All functions must be able to be performed with or without reasonable accommodation.
Typical Working Conditions:
This position operates in an office type environment as well as in the home and community of program participants. This position operates both indoors and outdoors on an ongoing basis. Exposure to the elements is common in this position. The employee may be exposed to heat/cold, steam, dust, cigarette smoke, pet dander, bacteria, noise, and communicable diseases.
