FOSTER CARE & ADOPTION CASE MANAGER.
The Foster Care & Adoption worker is responsible for the provision of all required adoptive home visits. The Foster Care & Adoption worker is responsible for being aware of and monitoring: the youth's emotional stability in the adoptive home, the quality of the relationship between the youth and the adoptive parents, and the youth's feelings regarding the termination of their family of origin and the current circumstances related to their adoptive placement. The worker is responsible for the development of treatment plans and coordinating the input of the youth, the adoptive parents, and the court into the treatment plans. The worker is to provide support and guidance to the parties involved in the treatment plan. The worker is also responsible for meeting with adoptive families according to the treatment plan and for coordinating services to aid in a successful adoption. The worker is expected to work with the youth's school and adoptive parents to ensure knowledge of progress or needs within the school system. The Foster Care & Adoption worker is expected to attend all court hearings and give all Adoption progress reports to the court system and DHS in a timely manner and monitor adoptive family progress and attend staff meetings. The worker is responsible for maintaining their own files in compliance with the agency, state, and BCAL requirements for Adoption cases in a child placing agency. The worker is responsible for the maintenance of charting according to agency and state expectations. The adoption worker is responsible to inform the adoptive family of all services, resources, and assure the child assessments, family assessment, and all adoption related requirements are performed in the state mandated time frame.
OUTLINE OF ESSENTIAL JOB DUTIES
  • Assess and diagnose family functioning to formulate treatment plans, and adoption progress.
  • Formulate treatment goals, objectives and monitor ongoing progress in the case file.
  • Conduct individual and family home visits. Implement all treatment planning and monitor progress toward Adoption
  • Evaluate effectiveness of ongoing interventions and treatment planning and report them to staff.
  • Support Adoptive families regarding treatment issues and rights of youth. Secure specific services for adoptive families, and youth.
  • Attend all staff meetings, present new cases and provide updated case presentations on existing adoption cases.
  • Adoption Case management responsibilities that help to ensure such things as adequate school services and developmentally appropriate daily routines for the youth in the adoptive home.
  • Provide outreach assistance and services to adoptive families, school, and youth.
  • Provide after care services to continue supporting the adoptive placement and success.
  • Submit necessary adoption documentation in a timely manner and attend all court hearings.
  • Continue professional development through training, conferences and readings.
  • Provide 24-hour on-call crisis availability to adoptive families on a rotating emergency pager.
  • Participates in adoptive and foster parent training.
REQUIREMENTS
Fostering Futures requires that the Foster Care & Adoption worker have a minimum of a BA/BS Degree in social work, psychology or counseling, or a human service field. A working knowledge of systems therapy, structural family therapy and trauma-related therapy is also desired. The worker is expected to learn all state requirements involving the timing of all reports and paperwork in an adoption case. This is a full-time position.
