Mending Our Mistakes, Inc. (M.O.M.) is seeking a compassionate, organized, and dependable Lead Case Manager / Family Stabilization Coordinator to help support parents and justice-involved individuals working to rebuild stability in their lives and relationships with their children. This position serves as the primary coordinator for participant services and support plans. The Lead Case Manager will work directly with individuals experiencing barriers related to housing instability, transportation, employment, court involvement, family separation, child support issues, reentry challenges, and financial hardship. The ideal candidate is highly organized, able to build trust with participants, and committed to balancing accountability with encouragement and support. This role requires strong communication skills, professional documentation, problem-solving ability, and a belief that people are capable of growth and restoration. Lived experience with overcoming hardship, recovery, family reunification, justice involvement, or rebuilding after personal challenges is valued within our organization. Core Responsibilities 1. Participant Case Management Conduct participant intake assessments and identify barriers to stability Develop individualized service and stabilization plans Monitor participant progress toward program goals Maintain regular communication and follow-up with participants Coordinate referrals to community resources and partner agencies Assist participants with goal-setting, accountability, and problem-solving 2. Service Coordination Coordinate support services related to: housing employment transportation financial literacy parenting support recovery resources behavioral health referrals supervised visitation support Work collaboratively with peer mentors, housing staff, volunteers, and partner organizations Assist participants with navigating systems and appointments 3. Documentation & Compliance Maintain accurate participant files and case notes Document services, referrals, participant engagement, and progress Assist with grant reporting and data tracking requirements Ensure confidentiality and compliance with organizational policies Help maintain organized program records and reporting systems 4. Interviews may be conducted prior to award notification, with final hiring decisions anticipated in July or August 2026. This position helps ensure participants are seen not simply for their past mistakes, but for their potential to rebuild and move forward.Pay:
