Job Overview We are seeking a motivated, dedicated and compassionate Licensed Social Worker to join our dynamic healthcare team. In this vital role, you will provide comprehensive social work services to diverse patient populations, supporting their emotional well-being and facilitating access to necessary resources. Your expertise will help improve patient outcomes through personalized counseling and case management. This 1099 position offers an exciting opportunity to make a meaningful difference in individuals' lives while working in a collaborative and supportive environment. Supervision towards LCSW available. Duties Conduct thorough patient assessments, including diagnostic evaluations and mental health screenings, to identify individual needs and develop tailored care plans. Provide clinical counseling, including behavioral therapy, motivational interviewing, and psychotherapy, to support patients coping with a variety of mental health issues. Facilitate group therapy sessions and conduct crisis intervention to address urgent patient needs effectively. Manage case documentation accurately within electronic health records (EHR) systems while ensuring compliance with HIPAA regulations for patient confidentiality. Collaborate with healthcare providers, family members, and community resources to coordinate comprehensive care plans . Familiarity with EMR systems and CPT coding for documentation purposes; experience with ICD-10 coding is preferred. Strong background in crisis management, diagnostic evaluation, and case management across diverse patient populations.
