Job Description

Join our dedicated team as an Intensive In-Home Counselor, where you will play a vital role in delivering comprehensive mental health and behavioral support directly within clients' homes and communities. This dynamic position offers the opportunity to work closely with children, adolescents, and families to promote positive change, resilience, and well-being. As an integral part of our multidisciplinary team, you will utilize evidence-based practices to assess needs, develop tailored treatment plans, and provide ongoing therapeutic interventions that foster growth and stability. Your energetic approach and compassionate mindset will empower clients to overcome challenges and achieve their goals in a supportive environment. Conduct thorough patient assessments to identify behavioral health needs, including diagnostic evaluations for individuals with autism or developmental disabilities. Deliver individual and group counseling sessions utilizing techniques such as cognitive-behavioral therapy (CBT), behavioral therapy, motivational interviewing, and art therapy to address mental health concerns like PTSD, anxiety, depression, and crisis situations. Implement behavior management strategies and crisis intervention techniques to de-escalate conflicts and ensure safety during home visits. Collaborate with families, caregivers, educators, and case managers to develop personalized care plans that incorporate special education strategies and applied behavior analysis (ABA). Maintain accurate medical documentation, including progress notes, treatment plans, and records compliant with HIPAA regulations. Collect data on client progress through systematic data collection methods to inform treatment adjustments and measure outcomes effectively. Provide education on mental health topics, medication management (if applicable), and coping skills to empower clients and their support systems. Valid licensure or certification as a Licensed Social Worker (LSW), Licensed Associate Counselor (LAC), Licensed Clinical Social Worker (LCSW), or Licensed Professional Counselor (LPC). Proven experience working with children and adolescents in home-based or community settings; experience with individuals with developmental disabilities or autism is highly desirable. Strong knowledge of behavioral health practices including crisis management, conflict resolution, trauma-informed care, and PTSD care. Familiarity with medical terminology, medical records management, patient assessment techniques, and healthcare privacy standards such as HIPAA. Excellent skills in case management, diagnostic evaluation, patient care coordination, and data collection methods. Proficiency in therapeutic modalities such as applied behavior analysis (ABA), cognitive-behavioral therapy (CBT), social work principles, and behavioral health interventions. Ability to manage multiple cases effectively while demonstrating flexibility in a fast-paced environment. Passion for working with diverse populations including individuals with disabilities or behavioral challenges; experience in special education is a plus.
