Position Overview:
We are seeking an empathetic and skilled Hospice Social Worker to provide emotional and practical support to patients and families navigating the challenges of terminal illnesses in the Santa Fe/Espanola surrounding area. The ideal candidate will assess needs, advocate for resources, and collaborate with interdisciplinary teams to deliver exceptional care tailored to individual situations.
Key Responsibilities:
Assess patient and family psychosocial needs and develop individualized care plans. Facilitate access to community resources, benefits, and services. Serve as an advocate for patients and families within healthcare systems. Collaborate closely with hospice team members to ensure comprehensive care. Provide bereavement and grief counseling, as needed. Document services and interventions in accordance with regulatory requirements. Communicate with the client, family members, and other healthcare professionals. Use holistic and alternative therapies as appropriate. Provide emotional support to clients, family members, and other healthcare professionals.
Qualifications:
Master's degree in Social Work (MSW) from an accredited institution. Licensure as a Social Worker (LMSW or LCSW) in the state of New Mexico. Experience in hospice, palliative care, or healthcare settings is preferred. Exceptional communication, problem-solving, and advocacy skills. Ability to support patients and families in emotionally charged situations with compassion. Knowledge of community resources and social service programs in Albuquerque.
