Pasadena, CA 91101 (Hybrid) As a MSW Social Worker, you will: We are seeking a compassionate and detail-oriented MSW Case Manager to join our Continuing Care team. This role focuses on delivering high-quality, patient-centered care coordination while supporting diverse populations, including Medical and Medicare members. The ideal candidate brings a strong clinical foundation, excellent communication skills, and the ability to think critically in a fast-paced, collaborative environment. Although the role is primarily remote, candidates must be available to pick up equipment and attend one day of onsite training in Pasadena. Key Responsibilities Conduct comprehensive case management activities using a holistic, patient-centered approach Develop and implement basic care plans tailored to individual member needs Coordinate care across multidisciplinary teams to ensure continuity and quality outcomes Identify and respond appropriately to urgent or emergent situations Engage with members through active listening, asking insightful questions, and adapting communication as needed Support members in navigating healthcare systems, including Medical/Medicare services Collaborate with internal teams and proactively seek guidance when needed Document interactions, care plans, and progress accurately and timely Requirements Master of Social Work (MSW) required (Candidates without MSW may be considered if they hold a valid LCSW) Strong customer service and interpersonal skills Ability to think critically, prioritize, and adapt in a dynamic environment Excellent communication and listening skills Ability to work both independently and collaboratively within a team Basic knowledge of care planning and case management processes The ideal candidate will have: A Master of Social Work (MSW) or LCSW, strong critical thinking and communication skills, a patient-centered approach to care, and experience supporting Medical/Medicare populations in a collaborative, fast-paced environment. Pay Range•: $27 - $32 per hour •Pay range offered to a successful candidate will be based on several factors, including the candidate's education, work experience, work location, specific job duties, certifications, etc.Benefits:
