Outreach Worker Early Interventionist (Early Childhood / Infant & Toddler Services)

Job Summary The Addison County Parent Child Center is seeking a compassionate and skilled Outreach Worker / Early Interventionist to provide Early Intervention services to infants and toddlers (birth-age 3) and their families. This role focuses on family-centered, routines-based intervention , including service coordination , developmental strategies , and specialized instruction that caregivers can implement in daily routines to strengthen children's functional, developmental, and learning skills . Responsibilities Provide Early Intervention services to infants and toddlers with developmental needs Deliver services using family-centered , routines-based , and evidence-based practices Coach, model, and educate parents, foster parents, and child care providers Support child development in: Social-emotional skills Language, communication, and early learning Adaptive, motor, and functional life skills Serve as Service Coordinator and primary point of contact for families Conduct developmental screenings, evaluations, and assessments Develop, implement, and monitor Individualized Family Service Plans (IFSP / One Plan) Coordinate referrals with healthcare, education, social services, and community providers Ensure compliance with Part C of IDEA , Vermont CIS , and Special Education Rules Maintain accurate documentation, data reporting, and billing information Participate in multidisciplinary team meetings and community outreach Support families in self-advocacy, resource navigation, and access to services Required Qualifications Education Bachelor's degree in: Early Childhood Education, Special Education, Early Childhood Development OR related Human Services field (Social Work, OT, PT, Speech & Language) Experience Minimum 2 years of experience working with children under age 3 Experience strengthening children's functional, developmental, and learning skills . Experience in home visiting, early intervention, outreach, or family services preferred Certification / Licensing Vermont CIS Early Intervention Certification , Early Childhood Special Education endorsement, or eligibility to obtain Valid driver's license and ability to travel locally Skills & Knowledge Infant and toddler development Developmental screening and assessment tools Part C IDEA regulations IFSP / One Plan development Family-centered practice Early childhood special education Service coordination Community-based services Strong written and verbal communication Independent and team-based work Why Join Us Every child deserves the chance to grow up happy, healthy, and supported. Pay:

$58,700.00 - $61,500.00 per year
