Full Job Description Work Schedule:

Location:
Tri-County Area (Clackamas, Multnomah, Washington County) Organization All Smiles Community Oral Health is a nonprofit organization that provides preventative dental services in community settings to children in eligible schools across the tri-county area (Clackamas, Multnomah, and Washington Counties). All Smiles represents a unique partnership of Medicaid Dental Plans, Coordinated Care Organizations, and community partners. The organization functions as a "public health overlay," bringing together health systems and safety net structures to deliver a coordinated, community-based approach to care. Position Overview The Program Associate supports outreach, care coordination, and program operations to ensure the effective delivery of dental services. This role combines community engagement, referral management, data reporting, and administrative support. The Program Associate works closely with families, school staff, and community partners. Spanish proficiency is preferred to best serve the diverse communities we support. This is a part-time position, typically working 20-25 hours per week based on operational needs. Occasional evening and weekend availability is required for events. This position is non-exempt and does not include health coverage.

- Initiate, track, and update patient referrals in a timely manner
- Communicate with dental plans, school staff, and community partners
- Maintain referral logs and follow-up documentation
- Serve as a resource to families navigating dental care
- Ensure HIPAA compliance and protection of patient information Program & Office Support Assist with data reporting and analysis of services.
Build and maintain electronic class lists and patient charts. Manage inbound phone calls, emails, and deliveries. Maintain inventory and prepare supplies. Support marketing and social media initiatives. Collaborate with team members to support program success. Outreach & Community Engagement Participate in outreach events (e.g., school nights, health fairs, family nights) Conduct outreach calls and assist families with completing consent forms. Support school-based service days and logistics. Deliver or assist with oral health presentations. Identify community engagement opportunities. Prepare outreach materials and dental kits. Track and report outreach outcomes.

- Maintain confidentiality, adhere to standards, and consistently follow HIPAA, FERPA, and other compliance regulations.
- Participate in project-based work (e.g. marketing initiatives).

- Spanish proficiency preferred
- Degree in social work, public health, related field preferred, or equivalent experience.
- Experience with dental/medical terminology preferred.
- Strong written and verbal communication skills.
- Highly organized and detail-oriented.
- Proficiency in Microsoft Office 365.
- Valid driver's license and reliable transportation.
- Ability to pass background and reference checks.
