Care Coordination Social Worker II (Bilingual) πŸ“ Hybrid - Santa Barbara, CA (In-office 1-2x/month/quarter) πŸ’² $40-$43/hour | πŸ—“οΈ Contract-to-Hire (2 months) Overview We are seeking a Care Coordination Social Worker II to join a collaborative healthcare team supporting complex patient populations. This is a contract-to-hire opportunity covering an employee on modified hours, with strong potential for long-term placement. This role is ideal for a clinically experienced, bilingual social worker who is passionate about patient advocacy, crisis support, and care coordination. Schedule Hybrid role 1-2 in-office visits per month in Santa Barbara Candidates outside Ventura/Santa Barbara/SLO (but within CA): 1 visit per quarter Key Responsibilities Conduct comprehensive patient assessments Provide care coordination and crisis intervention Collaborate with community agencies and support programs Maintain accurate case documentation and reporting Advocate for patient access, resources, and rights Present cases during multi-disciplinary complex case conferences Support patients and families navigating healthcare and social systems Required Qualifications (Must-Have)
Bilingual:
English & Spanish Master's degree in Social Work, Clinical Psychology, Psychology, or related field 5+ years of clinical experience Clinical experience includes hands-on patient work: assessments, crisis intervention, and care coordination in healthcare or behavioral health settings LCSW = counts as 2 years of experience Strong experience with: Comprehensive health assessments Crisis intervention Care coordination Community agency collaboration Reporting protocols Presenting in multi/interdisciplinary case conferences Why Apply? Job Type & Location This is a Contract position based out of Santa Barbara, CA. Pay and Benefits The pay range for this position is $50.00 - $55.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
