Day | Full-time | Monday to Friday 8 a.m. to 4:30 p.m.
Salary range:
$21.63 to $30.15 How you'll make an impact in this role: Guide families smoothly from referral to enrollment by conducting initial intake assessments, understanding their routines and concerns, and ensuring all state and federal eligibility timelines (including the critical 45-day enrollment window) are met. Coordinate multidisciplinary developmental evaluations by scheduling and participating in eligibility assessments alongside the provider team, delivering care directly in the home or the family's preferred community location. Build and execute individualized family support plans by connecting families with essential health resources, scheduling medical appointments, and securing dedicated service providers to match each child's unique needs. Manage a dedicated, long-term caseload of approximately 60 cases, serving as their primary advocate by monitoring mandatory quarterly, bi-annual, and annual evaluation timelines throughout their entire enrollment journey. Maintain precise, compliant documentation by capturing comprehensive case management notes and processing Medicaid billing to secure continuous funding and support for client services. Registration:
Finger Printing obtained prior to hire date or job transfer date required.
Education:
Bachelor's degree or higher from an accredited university with an emphasis in the areas of psychology, social work, health education, interdisciplinary sociology, early childhood, child development, special education or an equivalent degree based on transcript review. OR unrelated degree with a minimum of 3 years of documented experience with young children and families in a teaching, case management or counseling role or with individuals with special needs and/or developmental delays of any age in a case management role. What additional requirements you'll need: Targeted Case Management & Medicaid Billing Experience Early Intervention or Community Health Experience. Reliable Transportation and Regional Mobility.
