Position Summary Under the general supervision of the Senior Center Director and Police Chief, the Social Worker is responsible for providing case management support and advocacy for older adults to help them remain independent in their homes as long as possible. This position works collaboratively with the Dane County Aging & Disability Resource Center (ADRC) and other community partners to assess individual needs and connect older adults with appropriate services, including home-delivered meals, adult day care, long-term care programs, and other supportive resources. Additionally, the Social Worker works in close partnership with the Middleton Police and Emergency Medical Services (EMS) departments to support community members of all ages who are experiencing challenging situations and/or circumstances. This position will assess and identify the community members underlying needs and connect them with appropriate resources aimed at addressing and resolving their situations. Salary Information $62,909 - $67,627 annually; dependent on qualifications. Job Details A regular, Monday - Friday, 40-hour work schedule is required for this position, split between the Middleton Senior Center and Police Department. Work outside of regularly scheduled hours, including evenings and weekends, may be required to fulfill the essential functions of the position. Attendance at evening meetings and special events may be occasionally required. Remote work opportunities may be available with supervisor approval. Qualifications Minimum Education and Experience The requirements listed below are necessary to perform the essential duties of the position. 1. Graduation from an accredited college with a bachelor degree in social work, psychology, sociology, or a closely related field 2. Certified Social Worker (CSW) 3. A minimum of three (3) years of experience in social work or a closely related field 4. Access to personal or public transportation for job-related duties Other Qualifications The qualifications listed below are preferred, but not required to perform the essential duties of the position. Experience working with and completing needs assessments for older adults The City of Middleton recognizes that candidates may bring a variety of backgrounds and experiences. 6.
