Job Description

Division/Program Summary:
The Division of Disease Control's mission is to safeguard the health of New Yorkers through identification, surveillance, treatment, control, and prevention of infectious diseases, which is achieved through varied and interrelated endeavors of its seven Bureaus. The Bureau of Hepatitis, HIV, and Sexually Transmitted Infections' (BHHS) mission is to improve the lives of New Yorkers by ending transmission, illness, stigma, and inequities related to viral hepatitis, HIV, and sexually transmitted infections. BHHS accomplishes its mission through testing initiatives; prevention, care, and treatment programming; epidemiology and surveillance; training and technical assistance; community engagement; social marketing; policy advocacy; and racial equity and social justice initiatives. The ACE (Assess. Connect. Engage.) Team is responsible for providing partner services to New Yorkers newly diagnosed with HIV each year, including the identification, tracing, notification and HIV-testing of their sex and needle-sharing partners. ACE staff ensures that all HIV-diagnosed persons are linked to HIV clinical care.Job Duties and Responsibilities:
- Assist or guide ACE staff with complex case investigations and contact tracing.
- Counsel HIV-diagnosed persons on
HIV/AIDS
test results, transmission risk, and partner notification.- Assist HIV-diagnosed persons with disclosure of their HIV status to their partner(s) and provide education on HIV PrEP/PEP.
- Assist HIV-diagnosed persons with linkage to HIV clinical care.
- Link HIV-negative partners to PrEP providers for evaluation, counseling, and treatment.
- Identify barriers to care and connect clients to supportive services, mental/behavioral health, and substance misuse resources.
- Educate clients on ongoing access to HIV prevention and ancillary services.
- Promote the knowledge of New York State HIV-related laws and regulations among New York City providers.
- Identify citywide and community resources, establish, and maintain linkages with community organizations and partners.
- Collect data on HIV cases to fulfill surveillance and case investigation data requirements, including medical record review, patient, and provider interviews.
- Participate in ACE case management cohort review meetings and provide input to assist with case and contact investigation. Why you should work for us:

- 52613 Minimum Qualifications Qualification Requirements A Master's Degree in Social Work from an accredited school of social work.
- Patient interview, contact tracing/partner notification, medical record review, and community outreach experience.
- Excellent written and oral communication.
- Fluent English and either Spanish or French/Creole preferred.
- NYS Driver's License highly desirable.
- Must be able to work alternative hours (evening and weekends).
