Job Description

We're seeking a Behavioral Health Care Manager (BHCM) to deliver Collaborative Care Model (CoCM) behavioral health services in nursing home settings. This hybrid role includes on-site patient engagement and remote follow-up, under the supervision of a psychiatric consultant and in partnership with primary care providers. Ideal candidates are certified MAs with behavioral health experience or licensed mental health professionals (LMSW, LCSW, LMHC) comfortable working with geriatric populations. Key Responsibilities Identify residents with behavioral health needs in collaboration with facility staff and providers Administer validated screenings (PHQ-9, GAD-7) both in-person and remotely Deliver brief behavioral interventions and track symptom progress over time Conduct follow-up and monitor patient engagement and response to care Participate in case reviews with psychiatric consultants and apply recommendations Coordinate with families, facility teams, and providers to ensure care plan adherence Document all interactions in the EHR and meet CMS CoCM billing requirements Attend weekly team huddles and maintain consistent communication with supervisors Requirements
MA/LPN/RN
certification with geriatric or long term care experience or active mental health licenses such as LMSW, LCSW, LMHC, or MSW Experience with brief interventions, collaborative care, or geriatrics preferred Strong interpersonal skills and comfort working with older adults Organized, detail-oriented, and able to work independently Familiar with PHQ-9/GAD-7, and EHR systems Reliable transportation for on-site visits and a HIPAA-compliant remote work
