Job Description:
Under direction of the Director of Early Childhood and Chief of Student Support Services, the Social Worker will lead transition conferences, referral reviews, eligibility, and IEP meetings for children who are aging out of Early Intervention.
EDUCATION, TRAINING, AND EXPERIENCE
Masters or equivalency in Social Worker Experience with administration of Vineland 3, Connors 3 and completing social histories, and must have in depth knowledge of RIELDS, having participated in at least one of the RIELDS trainings offered by the Center for Early Learning Professionals.and participated in COS (Child Outcome Summary) training.
Must have experience with completing transition meetings and referral reviews for children that are currently in EI.
Must be able to develop goals and complete all paperwork that is involved with completing an early childhood referral.
Ability to develop and implement appropriate goals and objectives for each individual student based on the RIELDS.
Experience in being a case manager for PK children and corresponding to case load needs Working knowledge of Skyward and PowerSchool for documentation of evaluations, eligibility, and IEP placements
RIDE Requirements:
Current Rhode Island Teacher Certificate(s) in appropriate subject area
PK-12 Social Worker
Bilingual Endorsement preferred
ESL Preferred
DESCRIPTION OF DUTIES, RESPONSIBILITIES AND CURRICULAR LINKAGES
Provide written reports on students that have been evaluated; Participate in Transition conferences, referral reviews, eligibility meetings and IEP meetings.
Documentation of services for Medicaid purposes Ability to work together as a team to determine testing and meeting needs
The team works toward completion of pre-kindergarten cases All team members will work on completing evaluations File review to ensure appropriate permission has been obtained for completion of evaluations Contacting parents (phone calls and letters) to schedule evaluation sessions Documentation of all attempts to contact families and results of that contact in powerschool
Completion of documentation required for eligibility, inclusive of prior written notice of eligibility Input of all necessary documentation in Skyward and PowerSchool Must commit to work all of the days indicated in the posting Any/all related duties that may be assigned by Manager of Special Education and Services and/or Executive Director of Special Education
MINIMUM QUALIFICATION STANDARDS
Good communication and human relations skills needed
Formulate concise clinical reports.
Demonstrated proficiency with PC-based computer applications, Microsoft Office applications, email, web applications, and industry specific technology
