The Family Navigator provides person•and family-centered support to children and families by assisting them in accessing and coordinating services across healthcare, behavioral health, education, and community systems. This role focuses on advocacy, care coordination, and empowering families to effectively navigate complex service systems. The Family Navigator also supports educational advocacy and assists with planning and transitioning from children's to adult services to promote long-term stability and independence.Minimum Qualifications :
Bachelor's degree in Social Work, Human Services, Psychology, Education, or related field preferred (or equivalent experience) Experience working with children and families in community-based or social service settings Knowledge of local resources, family support systems, educational processes (IEP/504), and transition planning to adult services Strong communication, engagement, advocacy, and organizational skills Ability to manage multiple priorities and work independently Proficiency in documentation and basic computer systemsJob Responsibilities :
Conduct strengths-based assessments to identify family needs, goals, and barriers Connect families to community resources including healthcare, behavioral health, education, housing, and social supports Assist families in navigating service systems and completing applications or referrals Provide education advocacy support, including guidance around school systems, IEP/504 processes, and participation in school meetings when appropriate Support transition planning from children's to adult services, including linkage to adult providers, benefits, and long-term supports Develop and support individualized service plans in collaboration with families and care teams Provide ongoing follow-up, monitoring, and coordination of service Advocate on behalf of families to ensure access to appropriate supports across systems Support crisis prevention and problem-solving by addressing barriers to care Maintain accurate, timely, and confidential documentation Collaborate with internal staff, providers, schools, and community partners Participate in team meetings and contribute to coordinated care planning Develop relationships and connections with community providers in order to facilitate referrals and resource sharing.
