Benefits 401(k) Health insurance Paid time off Vision insurance Dental insurance Life insurance Full Job Description We are seeking a compassionate, experienced, and motivated Social Services team member to join our dedicated interdisciplinary team at a skilled nursing and rehabilitation center. The ideal candidate will advocate for residents' psychosocial well-being while ensuring compliance with all federal and state regulations. Responsibilities Develop and oversee the facility's social services program Conduct psychosocial assessments and participate in care planning Assist residents and families with adjustment, counseling, and discharge planning Facilitate and/or supervise resident programming, treatment planning, placement, and discharge processes Complete timely and accurate documentation in accordance with agency policies, procedures, contracts, and regulatory requirements Coordinate community resources and support services Participate in interdisciplinary care team meetings Ensure compliance with CMS, state, and facility regulations Address resident rights, grievances, and family concerns Maintain accurate documentation and resident records Frequently move throughout the facility, including interior and exterior areas Meet predetermined deadlines and reporting requirements Qualifications Bachelor's Degree in Social Work, Human Services, or related field is ideal, but not required Minimum of 2 years of skilled nursing facility or long-term care experience preferred Strong knowledge of Medicare, Medicaid, discharge planning, and resident rights Excellent communication, organizational, and leadership skills Compassionate, resident-centered approach to care Requirements Candidates must be able to: Pass a pre-employment drug screen and random drug screenings throughout employment Pass pre-employment and biennial criminal background checks Respond sensitively and competently to diverse cultural and socio-economic populations Work collaboratively with staff, service providers, and healthcare professionals Maintain professionalism and emotional control in a fast-paced environment Demonstrate computer literacy necessary for the position Work effectively both independently and as part of a multidisciplinary team Benefits Dental insurance Health insurance Life insurance Paid time off Vision insurance
