Job description: Job Summary We are seeking a compassionate and dedicated Hospice Licensed Social Worker to join our N. Charleston team. The ideal candidate will possess a strong background in social work, with expertise in providing emotional support, case management, and counseling to patients and their families facing end-of-life care. As a Hospice Social Worker, you will play a vital role in enhancing patient quality of life through comprehensive psychosocial assessments, advocacy, and personalized care planning. Duties Conduct thorough psychosocial assessments to identify patient and family needs, preferences, and challenges related to hospice care. Provide individual and group counseling sessions, utilizing techniques such as behavioral therapy, motivational interviewing, and clinical counseling to support emotional well-being. Collaborate with interdisciplinary teams to develop and implement personalized care plans that address social, emotional, and psychological needs. Assist patients and families with navigating healthcare systems, including hospice services, community resources, and legal considerations. Document all patient interactions accurately within electronic medical records (EMR systems), ensuring compliance with HIPAA regulations and medical documentation standards. Facilitate crisis intervention and provide support during difficult or emergent situations to ensure safety and stability. Engage in ongoing clinical research related to hospice care practices, dementia care, PTSD care, or working with individuals with disabilities to improve service delivery. Maintain up-to-date knowledge of medical terminology, ICD coding (International Classification of Diseases), CPT coding (Current Procedural Terminology), and relevant healthcare regulations. Qualifications Bachelor's degree in social work (BSW) or higher; licensure as a Licensed Clinical Social Worker (LCSW) or equivalent required. Proven experience in hospice care or palliative settings preferred; experience with inpatient, outpatient, nursing home, or assisted living environments highly desirable. Strong knowledge of medical records management, HIPAA compliance, ICD-10/ICD-9 coding, and medical terminology. Demonstrated skills in child & family counseling, behavioral health interventions, crisis management, and case management. Familiarity with EMR systems for data collection and documentation purposes. Experience working with diverse populations including children, seniors, individuals with developmental disabilities or autism spectrum disorder (ASD), dementia or Alzheimer's disease. Excellent communication skills combined with cultural sensitivity; ability to work effectively within multidisciplinary teams. Knowledge of hospice care principles, mental health counseling techniques such as cognitive behavioral therapy (CBT), motivational interviewing, and psychotherapy approaches.
