General Description:
The primary responsibility of the Case Manager is to provide support, guidance and resources for students in local school settings. This includes participating in daily screening activities at various school sites, working closely with other Wellness Program staff, and conducting case management activities throughout the school year at their assigned campus(es). Case Manager will be responsible for effectively communicating with parents, clinicians, students, and school staff. Flexibility and ability to travel between/within school districts is a must.
DUTIES/RESPONSIBILITIES
  • Administer Wellness Screening to assigned students/campus. Work closely with campus personnel to schedule and provide access to services
  • Develop daily, weekly, and monthly goals for program and students.
  • Respond and communicate with school staff and parent questions about Wellness Program
  • Review Wellness Screening data for follow up with students
  • Provide Risk Assessment follow ups: support students with suicidal ideation
  • Provide case management according to Wellness Program procedures: Support students showing signs of anxiety, stress, depression, suicidal ideation, social determinates to their health, peer interactions, social development
  • Connecting families to community resources
  • Document case notes within Data Management System in a timely manner
  • Participate in Weekly Consultations, Monthly Staff Meetings and other meetings as appropriate to position
  • Follow safety protocol for at-risk and safety situations, reporting when necessary
  • Maintain confidentiality with students, families, and staff
  • Travel to various Southeast Texas school districts
KNOWLEDGE/ABILITIES/SKILLS FOR SUCCESS
  • Assertive and direct communication skills
  • Well-developed listening skills
  • Excellent organization skills with attention to detail
  • Well versed in the Wellness Program policies, procedures and best practices
  • Well versed in mental health resources available in the community
  • Ability to communicate difficult information in a confident and compassionate manner
  • Persistent follow through to see that families are connected to recommended resources
  • Ability to problem solve in the moment and perform at an elevated level of independence
  • Ability to remain calm during times of crisis, and follow proper protocol for urgent evaluations
  • Knowledge of local resources Qualifications Bachelor's degree required : Examples but not limited to Social Services including Social Work, Psychology, Education and other Counseling related fields of study.
