Flexible Spending Account Essential Duties and Responsibilities:

Duties may vary based on specific program and position, and may include but are not limited to the following:

• Assist individuals in identifying and articulating their needs for services and supports.
• Coordinate, negotiate, and facilitate a comprehensive array of services and supports to address identified goals and desired outcomes.
• Provide behavioral intervention services, training, and support tailored to the needs of the assigned caseload.
• Ensure that planned services and supports are effectively implemented and properly documented.
• Connect individuals with appropriate community resources, services, and supports as needed.
• Maintain accurate and timely documentation of all meetings, progress notes, and activities related to individual service plans.
• Primary responsibilities include screening, intake assessments, crisis intervention, development of service plans and the implementation and monitoring of services for consumers. • Proficient in the use of computers and electronic health record (EHR) systems.

Qualifications Education and Experience:

• Bachelor's or Master's degree in a Human Services-related field such as: o Psychology o Sociology o Social Work o Rehabilitation o Counseling o Human Growth and Development o Gerontology o Special Education o Educational Psychology o Early Childhood Education o Early Childhood Intervention
• Licensed Registered Nurses are also eligible for this position

Certifications, Licenses, and Registrations:

• Valid Driver's License required
