  • Job Description:
  • Provides guidance to the social work team to ensure effective delivery of psychosocial services.
Evaluates the psychosocial needs of patients and families to develop individualized care plans. Develops and implements comprehensive psychosocial care plans based on patient and family needs. Conducts initial assessments at patient residences to identify psychosocial needs. Assists patients and families with obtaining financial resources and support. Coordinates referrals to community resources and services to address financial and social needs. Provides education and assistance in completing Advance Directives. Offers individual and group counseling to patients and families to address emotional and social challenges. Interacts with case managers to ensure appropriate hospice services are provided. Maintains accurate and timely documentation of patient interactions and care plans. Participates in interdisciplinary team meetings to collaborate on patient care. Other duties as assigned.
  • Knowledge, Skills, and Abilities:
  • Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements. [Required]
  • Knowledge of electronic medical record. [Required]
  • Ability to follow standard precautions and uses personal protective equipment when indicated. [Required]
  • Ability to complete Hospice of the Comforter Orientation and Training, as necessary. [Required]
  • Knowledge and understanding of advanced directives. [Required]
  • Knowledge of community resources. [Required]
  • Knowledge of Medicare/Medicaid process. [Required]
  • Education:
  • Bachelor's [Required]
  • Master's MSW [Required]
  • Field of Study:
  • Social Work
  • Work Experience:
  • 2+ years experience in the social work field [Required]
  • Healthcare/hospice experience [Preferred]
  • Additional Information:

  • Basic Life Support
  • CPR Cert (BLS) [Required]
