Job Description

Overview We are seeking a HOSPICE experienced compassionate and dedicated Social Worker to join our team. The ideal candidate will possess a strong commitment to improving the lives of individuals and families through effective counseling and support services. This role involves working with diverse populations, including children, families, and individuals with developmental disabilities. The Social Worker will provide therapeutic services, conduct assessments, and collaborate with other professionals to ensure comprehensive care. Duties Provide psychosocial assessments for hospice patients and families. Offer counseling and emotional support to patients, caregivers, and loved ones. Assist families in navigating end-of-life decisions, advance directives, and care planning. Coordinate community resources, financial assistance, and support groups as needed. Collaborate with the hospice team to develop individualized care plans. Document services in compliance with regulatory and organizational standards. Advocate for patients' rights and dignity throughout the hospice journey. Requirements Valid state licensure as a Social Worker (LCSW or equivalent). Experience working with individuals with developmental disabilities, or in geriatrics is preferred. Familiarity with clinical research methodologies and behavioral health practices. Proficiency in EMR systems for managing patient records effectively.
EMR:
Well-sky Consolo. Strong understanding of crisis management techniques and patient care protocols. Excellent communication skills for effective interaction with clients, families, and healthcare professionals. Knowledge of psychopharmacology as it relates to mental health treatment is a plus. Ability to work collaboratively within a team environment while maintaining a client-centered approach.
