  • 5pm Join Our Compassionate Care Team We are seeking a dedicated and compassionate Texas Licensed Social Worker to join our team at a skilled nursing and rehabilitation facility in Dallas.
The ideal candidate is passionate about advocating for residents, supporting families, and helping improve quality of life within a long-term care environment. Responsibilities Conduct psychosocial assessments for residents upon admission and as needed Develop and implement individualized care plans with the interdisciplinary team Provide emotional support and counseling to residents and families Assist with discharge planning and transitions of care Coordinate community resources and services for residents Advocate for resident rights and ensure compliance with state and federal regulations Maintain accurate and timely documentation Participate in care plan meetings and facility committees Support residents with adjustment, grief, behavioral, and family concerns Qualifications Current Texas Social Worker license required ( LBSW or LMSW ) Long-term care or skilled nursing experience preferred Knowledge of Medicare, Medicaid, and long-term care regulations preferred Strong communication and interpersonal skills Ability to work collaboratively with residents, families, and healthcare teams Compassionate, organized, and dependable Benefits Competitive Pay Health, Dental & Vision Insurance Paid Time Of Supportive Team Environmentf Apply Today Join a team that values compassion, dignity, and quality care.
