In this vital role, you will provide expert social work services across various clinical settings, including inpatient, outpatient, hospice, and community care environments. Your expertise will support patients and their families through complex emotional, psychological, and social challenges, ensuring comprehensive care that promotes well-being and recovery. This position offers an exciting opportunity to make a meaningful difference in the lives of diverse populations, including children, seniors, individuals with disabilities, and those facing mental health or behavioral health issues. You will collaborate closely with multidisciplinary teams to deliver personalized interventions, conduct thorough assessments, and develop tailored care plans that align with each patient's unique needs. Responsibilities Conduct comprehensive patient assessments to evaluate social, emotional, and environmental factors impacting health and recovery. Develop and implement individualized care plans incorporating evidence-based practices such as behavioral therapy, motivational interviewing, cognitive behavioral therapy, and applied behavior analysis. Provide clinical counseling services including individual, group, family, and crisis intervention sessions to support mental health and behavioral health needs. Facilitate intake processes for new patients by gathering detailed medical records, documenting medical terminology accurately within electronic medical record (EMR) systems while adhering to HIPAA regulations. Collaborate with healthcare providers to coordinate case management efforts, ensuring seamless delivery of services across inpatient, outpatient, hospice, assisted living, nursing homes, and home & community care settings. Support patients with specialized needs such as dementia care (including Alzheimer's), autism spectrum disorder (ASD), PTSD care, developmental disabilities, and geriatrics through targeted interventions. Collect data for clinical research projects related to social work practices or patient outcomes; maintain meticulous medical documentation including CPT coding and ICD coding for diagnoses. Engage in crisis management by providing immediate support during emergencies or behavioral crises while maintaining safety protocols. Participate in ongoing professional development related to medical terminology, ICD-9/ICD-10 coding updates, HIPAA compliance, and emerging best practices in behavioral health. Qualifications Master's degree in Social Work (MSW) from an accredited institution; licensure as a Licensed Clinical Social Worker (LCSW) or equivalent required. Proven experience working in hospital settings or inpatient units preferred; experience in hospice care or senior/geriatrics is highly valued. Strong knowledge of mental health counseling techniques including behavioral therapy, motivational interviewing, crisis intervention, and psychotherapy modalities. Familiarity with EMR systems for data collection and medical documentation; proficiency with
ICD-9/ICD-10
coding essential. Ability to perform comprehensive patient assessments using clinical research methods; experience working with individuals with disabilities or developmental disorders is a plus. Excellent communication skills for engaging with diverse populations such as children (special education), adults in outpatient clinics or assisted living facilities, and elderly patients with dementia or Alzheimer's disease.
