Licensed Social Worker

Job Summary:

Responsibilities:

Provide emotional support, counseling, and resource navigation to patients and families in their homes or care facilities. Conduct psychosocial assessments and develop individualized care plans in collaboration with the interdisciplinary team. Assist with advance directives, end-of-life planning, and grief support. Travel within a designated territory to visit patients (mileage reimbursed). Serve as a liaison between families and community resources.

Requirements:

Bachelor of Social Work or Master's degree in Social Work from an accredited program. Current state licensure LBSW, LMSW, or LCSW Strong communication, advocacy, and crisis intervention skills. Valid driver's license and reliable transportation.

$50,000.00 - $60,000.00 per year
