In this vital role, you will provide compassionate clinical support, conduct thorough assessments, and develop personalized care plans to promote well-being and recovery. This position offers an engaging opportunity to work across diverse settings, including outpatient clinics, inpatient facilities, hospice care, and community-based programs. Your dedication will help empower clients to achieve their goals and improve their quality of life through expert counseling and case management. Duties Conduct comprehensive patient assessments to evaluate mental health status, behavioral patterns, and social needs using clinical research methods and diagnostic tools such as ICD-10 coding. Provide individual, group, and family counseling sessions utilizing evidence-based techniques like cognitive behavioral therapy (CBT), motivational interviewing, and behavioral therapy to support mental health recovery. Manage case documentation meticulously within electronic medical record (EMR) systems while ensuring compliance with HIPAA regulations for patient confidentiality. Collaborate with multidisciplinary teams—including medical professionals, hospice staff, educators, and community organizations—to coordinate patient care plans tailored to specific needs such as Alzheimer's care or autism spectrum disorder. Facilitate crisis intervention and crisis management strategies for clients experiencing acute mental health episodes or behavioral emergencies. Assist with intake procedures by gathering detailed medical histories, conducting diagnostic evaluations, and establishing treatment goals aligned with patients' developmental disabilities or geriatrics needs. Support research initiatives related to behavioral health or clinical studies by collecting data, analyzing outcomes, and contributing insights that advance best practices in social work and mental health counseling. Experience Proven experience working in clinical counseling environments such as outpatient clinics or inpatient settings with a focus on mental health or behavioral health services. Hands-on experience in hospital or nursing home settings providing patient care for diverse populations including seniors, children, or individuals with disabilities. Familiarity with medical terminology, CPT coding for billing purposes, ICD coding systems (ICD-9/ICD-10), and medical documentation standards required for accurate record keeping. Demonstrated ability to perform intake assessments, diagnostic evaluations, and case management within hospice care or post-acute care contexts. Knowledge of working with individuals with developmental disabilities or neurodegenerative conditions such as dementia or PTSD is highly desirable. Experience utilizing EMR systems for data collection and maintaining detailed medical records while adhering to HIPAA guidelines. Strong skills in crisis intervention techniques combined with the ability to handle sensitive situations involving patients at various stages of mental health recovery or crisis situations involving substance abuse or addiction counseling.
