TheSocial Worker assists with the acclimation of admission for each resident as well as discharge planning upon leaving the facility. They provide direct resident and family support and acts as a resident advocate while working collaboratively with our nursing and management team to provide the best experience possible. Social Worker Qualifications:

High School Diploma or equivalent Within the last five years, must have one year of supervised social work experience (preferably in a healthcare setting) Must have valid Virginia driver's license
