Job Description

Infant Service Coordinator - PRN District 19 Community Services Board Overview Description District 19 Infant Intervention Program is seeking a qualified professional to assume the responsibilities of PRN Infant Service Coordinator. The Infant Service Coordinator is responsible for overseeing all collaborative requirements of service delivery for children birth to age 3 years who exhibit a developmental delay, atypical development and/or have a diagnosed condition that has a high probability of resulting in a developmental delay. The Service Coordinator is responsible for coordinating and implementing the assessment for service planning, facilitating the development, review and evaluation of the Individualized Family Service Plan (IFSP), coordinating and monitoring the delivery of services, coordinating with medical and other services involved with the child, and facilitating the development of a transition plan for when the child leaves early interventions services. District 19 Infant Intervention Program services the communities of Colonial Heights, Hopewell, Prince George, Petersburg, Dinwiddie, Sussex, Surry, Emporia, and Greensville County. On the job travel is required. Successful applicant must have a minimum of a Bachelor's degree in Human Services field (which includes social work, gerontology, psychology, psychiatric rehabilitation, special education, sociology, counseling, vocational rehabilitation, and human services counseling) or education field. Basic computer skills. Applicant must have knowledge of and ability to identify community resources and activities to support service delivery and the family's participation and inclusion; Part C regulations; skills to include coordinating and monitoring services provided to infants and families. Must have experience working w/infants & toddlers who may have developmental delays or experience related to family services. Must pass all required training modules for Early Intervention (EI) certification, including EI Targeted Case Management, within 6 weeks of hire.
Preferred qualifications:
knowledge of service delivery, including coaching, to infants/toddlers with developmental delays; knowledge of child development; ability to coordinate and facilitate multidisciplinary team activities; skills in conflict resolution; demonstrate effective written, oral, and interpersonal communication principles and techniques; knowledge of general principles of client record documentation and specific requirements of the Part C system; skills in building rapport with families, including communication, listening, and problem solving with families from diverse socio-economic backgrounds; ability to identify and coordinate community activities to support the family's participation and inclusion. A bilingual (English/Spanish) candidate is preferred.
Salary:
Position is a grade 208. Starting hourly wage is $24.60/hour. Offers will be commensurate with knowledge, skills, and abilities subject to budgetary constraints.
Hours:
Monday - Friday, 8:30am - 4:30pm
Telework:
This position is not eligible for telework.
