Serves as a liaison to and works closely with health care professionals on provision of available services and resources the command to ensure best possible care in accordance with community, nional and Marine Corps standards. Facilites referrals to approprie specialists. Assists in identificion of community resources. Manages and monitors the member¿s progress from enrollment to assessment. Personally contacts each EFMP family upon notificion of assignment to offer support services. Conducts interviews with clients and their families. Establishes and maintains a case file for each assigned EFMP family th includes the Installion Support Privacy Consent Form and detailed contact notes. Assesses individual client and family circumstances upon and during enrollment to track progress of activity and provide upded informion on services to improve quality of life. Serves as an advoce for and assists Marines and their families in accessing available resources and services to support individual needs. Participes in meetings between Marine families and special needs support professionals to develop the Individual Educion Plan (IEP). Assist families in assertively pursuing rights to resources available to them. Prepares evaluions and recommendions to assist eligible members in connecting with approprie resources and services. Assists in coordining referral for care and resources with internal and external referral sites in the local area. Educes referral sources, community agencies and service organizions regarding scope and management of the EFMP. Maintains a detailed and consolided portfolio of services for special needs families the command and in the local community. Participes in coordinion and execution of special needs camps, support groups for EFMP families, special events and workshops coordined by the EFMP. Assists in checking th EFMP staff complete training in compliance with Marine Corps Order (MCO) 1754.4. Maintains personal records of staff training completion for review by the respective manager. Participes as assigned in the Special Needs Evaluion and Review Team (SNERT) as outlined in
MCO 1754.4.
Participes in the maintenance and development of a library of special needs educional, referral and resource merials. Works with assigned families to coordine use of the EFMP Respite Care Program. Works in concert with the EFMP team to ensure the program works effectively to support families while maintaining compliance with all legal and regulory guidance. Organizes work and sets short and long term priorities. Prepares reports of stus of activity on enrollment of members. Conducts quality assurance activities and studies to analyze, evalue and provide recommendions on program issues. Screens enrollment and updes Forms DD-2792, DD2792-1 and other supporting medical documention reled to case evaluion, assessment and therapy. Identifies and acquires conflicting or incomplete case informion to provide proper recommendions. Determines stus and eligibility of enrollment of members. EVALUATIONS
Qualificions Bachelors degree in a Behavioral Health or a reled social science, or combinion of equivalent educion and experience, or four years of approprie experience th demonstres knowledge of one or more of the behavioral or social sciences and working in a case management capacity with families who have members with special needs. Knowledge of the Military lifestyle to include service/unit organizion and rank structure and military, family and personal support services available. Knowledge of military laws, policies, regulions, resources and practices regarding medical and educional benefits, programs, services and community agencies for family members enrolled in EFMP. Skills in listening and interviewing, and applying sound judgment for assessing individual and family needs; problem-solving and advocing for support or services; or determining an approprie course of action. Written and oral communicion skills to effectively provide case management assistance to persons in various emotional stes and varying needs; and to conduct briefs, workshops and training. Inter-personal skills to rele to all segments of the military and civilian community using persuasiveness, poise, tact and objectivity.
