Job Description

Overview We are seeking a dedicated and compassionate Social Worker to join our team. The ideal candidate will possess a strong background in adult and family counseling, mental health, and various therapeutic modalities. This role requires a commitment to providing high-quality patient care, conducting assessments, and developing effective treatment plans for individuals across diverse populations. The Social Worker will work collaboratively with healthcare professionals to support patients and their families in navigating complex emotional and social challenges. Responsibilities Conduct comprehensive patient assessments to determine needs and develop individualized treatment plans. Provide counseling, including individual and group therapy sessions, to include bereavement services, utilizing techniques such as cognitive behavioral therapy and motivational interviewing. Engage in crisis intervention and management to support patients and caregivers in acute distress. Collaborate with multidisciplinary teams to ensure holistic patient care, including case management and coordination of services. Maintain accurate medical documentation in compliance with HIPAA regulations, ensuring confidentiality of patient records. Participate in clinical outreach initiatives to enhance service delivery and improve patient outcomes. Facilitate group therapy sessions focused on death and dying counseling, and mental health support. Provide education and resources to patients and families regarding available services, treatment options, and community resources. Stay informed about current trends in social work practices, medical terminology, and relevant coding systems ( ICD-10). Support patients with special needs through tailored interventions for individuals with conditions such as mental illness, poor coping, Alzheimer's Disease or Dementia. Experience A degree in Social Work, Marriage and Family Therapist or Mental Health Counselor or a related field is required; Master's licensure preferred. Experience in child & family counseling, crisis intervention, and behavioral therapy is highly desirable. Familiarity with EMR systems for efficient data collection and management of medical records. Previous hospice, hospital or clinic experience is beneficial; experience working with geriatrics is a plus. Knowledge of hospice care practices, outpatient services, assisted living environments, and home & community care experience is advantageous. Strong understanding of diagnostic evaluation processes and case management principles. Ability to work effectively with diverse populations including individuals with autism or other disabilities.
