" Our organization provides services designed to help members of our community achieve their individual goals. MPNN BH has over 60 distinct programs to aid and support all ages and many situations. Job Summary Under general supervision, employee is responsible for providing counseling/crisis intervention/case management services, as part of a system of family preservation and support services, to those children and adolescents who may be at risk to themselves or others, or who may be at risk of hospitalization or other restrictive placement due to mental illness and/or severe behavioral problems and their families. Counseling/crisis intervention/case management services include client identification, assessment, planning, implementing and monitoring service plans for the purpose of facilitating lasting improvements in mental, emotional, and behavioral functioning, and advocacy and resource development. Employee provides crisis intervention and family preservation services on an intensive schedule determined by client need. Employee must exercise considerable initiative and judgment in the effective and efficient administration of departmental programs. Employee must also exercise tact and courtesy in contact with staff, clients, families, public, and service providers. Reports to the Coordinator of Youth Intensive Community Services. Job Responsibilities Provides counseling/crisis intervention/case management services, as part of a system of family preservation and support services, to those children and adolescents, at risk to themselves or others, or at risk of hospitalization or other restrictive placement due to mental illness and/or severe behavioral problems and provides those services to their families. Develops and implements treatment plans comprised of counseling interventions specifically designed to meet each individual client's needs. Works in collaboration with Coordinator of Youth Intensive Community Services and other Community partners, as defined by the state Comprehensive Services Act, to determine level of intervention required. Collaborates with the therapist who provides outpatient counseling services with the individual. Provides crisis intervention counseling to children, and families for short-term intensive periods, or for longer-term, less intensive periods, as determined by the needs of the consumer and family. Provides additional case management services, such as coordinating in-patient discharge planning, assisting clients in investigating educational opportunities, living alternatives, and education and entitlement opportunities as relates to family preservation and support services. Maintains a cooperative working relationship with agencies collaborating under the state Comprehensive Services Act, including juvenile court services units, departments of social services, schools, public health, and other public and private agencies and individuals involved with children at risk as defined by the Act. Keeps informed as to current literature and legislation related to the Comprehensive Services Act, family counseling, family preservation and support, and crisis intervention. Provides after-hours, holiday, and weekend coverage as part of the system of family preservation and family support services. Maintains complete, accurate, and current clinical charts and records. Participates in the training of new staff; makes informational presentations to staff and community organizations as requested. Completes initial inquiries; provides telephone consultations. Prepares statistical reports as requested. Attends staff meetings, training sessions, and seminars. Maintains Census information as required by funding sources. Performs other related work as required. Job Requirements Essential Must pass all required Background Checks. Must possess a valid Virginia Driver's License with reliable transportation and driving record acceptable to agency insurance carrier. Must obtain a TB test with satisfactory results within 30 days of employment, and yearly thereafter as required by licensing authority. Education and Experience Bachelors Degree in Human Services, Social Work, Psychology, or related field which prepares candidate QMHP status, and 1 year of experience in emergency/crisis services, family, individual or group counseling, family preservation, with children and adolescents which qualifies candidate for QMHP-C.
