We are seeking a Social Worker for our Tidewater Cove Assisted Living Facility Program Responsibilities and Duties for

TWC Social Worker:

Meet with administration, medical, and nursing staff, and other related departments in planning social work plans of care for residential consumers. Maintain knowledge of current Federal and State regulations, as well as, professional standards, and make recommendations accordingly for changes to policies and procedures. Attend Team meetings. Participate in discharge planning and consumer assessment and maintaining consumer family connections. Work with the program consultants to identify, implement and recommend changes to the consumer's social work plans of care. Developing social histories and relationship maps Coordinating with agencies for benefits as needed for consumers and surveying recipients for program satisfaction Provide problem solving solutions, financial assistance, discharge planning, and referrals to other community agencies when specialized assistance is required.

Qualifications and Skills:

Bachelor's from an accredited School of Social Work One (1) year related work experience; two (2) years preferred Valid Driver's License and acceptable DMV record at the time of employment and continuously thereafter Benefits for: Health Insurance/Life Insurance Retirement/Flexible Spending Account (FSA) Vacation/Sick leave Legal Resources VA529-
