Job Description

Hospice experience is a
MUST Responsibilities:
Provide Clinical Social Work to Hospice population. Complete initial and ongoing social evaluation of patient and family dynamics Participates in the coordination of services with other hospice team members and community resources as needed. Maintains and processes all documentation in compliance with agency standards.
Requirements:
Home Care, Hospice or Bereavement counseling experience Valid Driver's License, auto insurance and reliable transportation Compassion and integrity
