We build and lead integrated, multi-disciplinary clinical teams to provide support to clients and currently serve over 4,000 patients in multiple locations. To be successful in this role, you will have: The ability to establish and maintain a supportive relationships with children, youth and families. The coordination to facilitate team meetings, run groups, and work with staff and stakeholders. Knowledge on how to provide holistic and therapeutic case management services.
