Licensed Master Social Worker (LMSW) / MSW Case Management

Job Overview:

We are seeking a Licensed Master Social Worker (LMSW/MSW) for a hospital-based case management role in an acute care setting. The social worker will support adult and geriatric patients through care coordination, discharge planning, and psychosocial assessments while collaborating with interdisciplinary teams across multiple hospital units.

Guaranteed Hours:

72 hours per 2 weeks

Responsibilities:

Conduct biopsychosocial assessments and initial evaluations Coordinate discharge planning and post-acute care services Collaborate with physicians, nursing, and interdisciplinary teams Arrange home care, DME, and community resources Provide patient and family education and support Assist with advance directives and care planning Document all care coordination activities in Epic EMR Manage caseload of approximately 18-24 patients (up to 30 on weekends) Float across units and campuses as needed

Requirements:

Master's Degree in Social Work (MSW) Active Washington State Social Work License (LICSW, LSWAIC, LASW, or LSWAA) Minimum 1 year experience (acute care required) Experience in discharge planning and care coordination EPIC charting experience required BLS Certification BI02

Pay:
