Carease Health - WI As a Care Companion/Social Worker with Carease Health - WI, you will play a vital role in providing emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. Role:
As a Care Companion/Social Worker, you will: Visit designated facilities in accordance with the schedule set with your supervisor Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns Document visit notes and maintain detailed records of interactions and interventions after each visit Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric
NP Qualifications:
Associate Degree in Social Work, Psychology, Counseling, or another mental health-related field Understanding of geriatric mental health, crisis management, Counseling, Mental Health Assessments, and psychosocial support in long-term care settings Strong communication, documentation, and relationship-building skills
