Madison, WI Full Time 1.0 FTE (40 hours/week) | Day Shift | Monday-Friday | 8a-5p Includes on call 1 weekend/quarter, no weekends, and 2 holidays/year Make a lasting impact with Agrace Join Agrace, a nationally recognized nonprofit leader in hospice and palliative care. How You'll Make a Difference (Essential Functions) The Spiritual and Grief Counselor serves the interdisciplinary team by responding to referrals, developing and carrying out individualized care plans, and providing individual and group spiritual and grief support. Key Responsibilities Patient and Family Assessments Respond to referrals from clinical teams by contacting the patient and/or family to establish counseling relationship and expectations. Perform spiritual and grief assessment, intervention, and care planning to meet the individual needs of patients and/or families. Engage in ongoing assessment of care plans through interdisciplinary review in accordance with Federal and State Guidelines Work collaboratively with the patient's designated faith and spiritual community, and refer to other community resources as needed Provide Bereavement Support Develop an individualized bereavement plan of care to address the full scope of survivor needs. Provide updates to the interdisciplinary team regarding bereaved Connect bereaved with community resources to assist in the grieving process Lead Community Grief Groups and Memorials Develop, implement, and facility support groups for Agrace families/community members Provide supervision and support to counseling services volunteers Participate in the planning and coordination of Agrace community bereavement events Other Duties Participate in internal work groups, meetings and committees relative to position responsibilities Provide counseling support (through group sessions, memorial services, reflections, etc.) to the interdisciplinary teams and community partners You Bring (Qualifications) Degree in Social Work, Counseling (or other related counseling degree), Divinity, Theology, or Pastoral Care, or equivalent combination of education and work experience required At least one unit of Clinical Pastoral Education (CPE); (required unless degree in Social Work obtained) Two or more years' counseling experience preferred Valid driver's license, auto insurance, and access to a reliable vehicle Experience providing counseling and/or spiritual care in both a group and individual setting Experience working with grief as related to death Intermediate Microsoft Office Suite skills Ability to work effectively and cooperatively with others Experience using industry specific software Ability to communicate effectively verbally and in writing Ability to be flexible in time and task to meet organizational needs Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property Ability to work at various Agrace locations Ability to use proper body mechanics and standard precautions Benefits Competitive pay with mileage reimbursement Health, dental, and vision insurance for employees who are hired for 28+ hours per week (.7 FTE +) Retirement plans with employer matching Generous Paid Time Off (PTO) and holiday paid time off Comprehensive onboarding, mentorship, and career development About Agrace For more than 45 years, Agrace has been an innovator and leader in both caring for patients and their families, and in educating others about death, dying and grief.
