To learn more about CW Solutions, visit our website at www.changewithin.net. We are currently recruiting for a case manager in our NorthCentral FoodShare Employment & Training (FSET) Program. We partner with Wood County Human Services Department to operate NorthCentral FSET. If you have a passion to support the members of our community, please consider joining our team as an FSET Case Manager in Rhinelander , WI! This full-time, permanent position performs case management services to assess the needs, strengths, and barriers of FoodShare recipients who enroll in FSET and works collaboratively with participants to develop a plan that works to assist them in reaching their employment goal(s).
Key Responsibilities:
Provide holistic, inclusive and comprehensive services to all FSET participants including but not limited to initial and ongoing assessment of a participant's current situation, strengths and needs; development of goals; progress monitoring; assistance with job applications, resume development and interview preparation; soft skills development through individual and group activities; documentation of case actions; advocacy and referral. Ensure all FSET participants are provided opportunities to gain life skills, job skills and/or education credentials to enable them to achieve their career goals. Assess and provide support services to facilitate FSET participants being able to complete FSET activity, including employment, while enrolled in the program. Support services include gas cards, bus passes, interview clothing, work-related clothing and tools, vehicle repairs and short-term education costs. Develop and maintain community relationships to reach eligible individuals who may not have heard of FSET and to ensure duplication of services does not occur. Work collaboratively with providers to provide successful outcomes for participants. Document actions in a state computer system timely and utilize online reports to monitor and manage caseload for policy compliance and accuracy.

This includes providing culturally sensitive and appropriate support to individuals within the context of their identity, family, community, tribe, history, culture and traditions. Establish rapport and trust with participants to ensure they can engage in the program and feel respected and empowered to achieve their work-related goals, while maintaining a clear sense of boundaries and confidentiality. Ability to establish rapport with coworkers, partner agencies, Income Maintenance staff, and other applicable individuals and groups that are intersected with. This includes a strong sense of respect, honoring confidentiality, and a willingness to engage in crucial conversations to resolve conflict. Ability to work independently with a strong sense of focus. Excellent written and verbal communication skills, keyboarding and MS Office skills.
