Job Description

Company Overview Serenity for Life Center, LLC (SLC) is a social service and education center that helps individuals, pregnant women, teens, children, children with disabilities and families from all sorts of backgrounds. SLC will assist you with navigating through societal barriers and to empower and gain stability. Summary Serenity for Life, LLC is an approved Children's Long-Term Support Provider (CLTS). The Children's Long-Term Support (CLTS) waiver Program helps children with disabilities and their families through supportive services. As a Support Coordinator at Serenity for Life Center, LLC, you will play a vital role in the CLTS wavier program. As a Support Coordinator , you will work hands on with children who have some sort of disability. You will also work alongside with the child's caregiver or their family through supportive services, that will help the child grow, live their best lives in their home and community. We service Milwaukee county areas, Waukesha and some of the surrounding counites. SLC is strongly looking for male mentors The following Services being offered through this wavier are: Empowerment, Mentoring, Personal Support, In-home Respite, Daily Living Skills, Community Integration and Transportation (case-by-case) Responsibilities Provide compassionate support to the children and families in various settings, mainly in the comfort of their home. A child must be under 22 years old for the CLTS program. Assist clients with daily living activities to promote independence. Assist clients with personal supports to guide them and promote independence. Monitor clients needs and access if needed Educate clients and provide the necessary tools that promote well-being. Facilitate access to community resources and services as needed. Ensure compliance with resident rights and confidentiality standards. Participate in team meetings to discuss client, program progress and needs (if needed, depending on services). Maintain accurate documentation of client interactions and complete monthly service log, per client. Provide brief, intermittent, short time breaks (respite) for family/caretaker in the home, (case-by-case) Qualifications Experience in social services, human services, home health or patient care settings is required. Associate degree is recommended; however, experience will be considered ASL-Fluent is recommended CBRF is recommended Strong knowledge of child development, social service and behavioral exp. Strong knowledge of assisted living practices and patient monitoring techniques. Experience working with children, children with developmental, cognitive, physical and/or mental health. Certification in CPR is helpful but will trained if needed. Excellent communication skills to effectively interact with children, family and team members. Ability to work collaboratively within a multidisciplinary team environment. Have a reliable transportation Compassionate demeanor with a strong commitment to supporting families. Able to pass a background check and no prior CPS (substantiated ) reports.
