Job Description

Overview:
As a Case Coordinator, you will play a key role in supporting children, youth, and families by connecting them with services, coordinating care, and promoting positive outcomes through a person-centered approach.

In this role, you'll serve as a central point of contact for participants, families, and service providers, ensuring individuals receive the support and resources they need to thrive.
What You'll Do:
  • Serve as the primary point of contact for participants, families, and service providers
  • Coordinate services and supports for children and families using a person-centered approach
  • Gather and assess information to identify needs and appropriate resources
  • Provide education, guidance, and ongoing support to families and caregivers
  • Build and maintain relationships with community agencies and service providers
  • Identify service gaps and contribute to program development efforts
  • Attend required meetings and collaborate with internal teams
  • Maintain accurate, timely documentation and case records (within 3 business days)
  • Assist with referral screening and intake for children with disabilities
  • Prepare reports and documentation for court or program requirements Why Join Fundamentals?

What We're Looking For:
  • High school diploma or GED required
  • Associate's or Bachelor's degree in social work, psychology, human services, or related field (preferred)
  • Minimum of 2 years experience in health or human services (preferred)
  • Social Work certification (preferred)
  • Valid driver's license required
  • Strong organizational and documentation skills
  • Ability to manage multiple cases and priorities effectively
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Commitment to ethical and professional standards
Schedule & Program Details:
This position works Monday-Friday 11AM-7PM during the school year and transitions to daytime hours (8AM-4PM) during the summer months. Rotational on-call Saturdays are required year-round. Join us and make a meaningful impact.
Pay:
$48,000.00
  • $55,000.
