Job Description

Job Summary We are seeking a dedicated and compassionate Licensed Social Worker to join our dynamic healthcare team. In this vital role, you will provide comprehensive social work services to patients across various settings, including inpatient, outpatient, hospice, and community care environments. Your expertise will help patients and their families navigate complex emotional, social, and healthcare challenges, ensuring they receive personalized support that promotes well-being and recovery. This position offers an exciting opportunity to make a meaningful difference in people's lives while working within a collaborative and innovative team. Responsibilities Conduct thorough patient assessments to evaluate social, emotional, and environmental needs using clinical counseling techniques. Develop individualized care plans that incorporate behavioral health strategies, crisis intervention, and case management to support patient recovery and stability. Provide individual and group therapy sessions utilizing evidence-based practices such as cognitive behavioral therapy (CBT), motivational interviewing, and behavioral therapy. Assist patients with discharge planning, home & community care coordination, and navigating healthcare systems while adhering to HIPAA regulations for medical documentation and records management. Collaborate with multidisciplinary teams—including medical staff, therapists, and community organizations—to ensure holistic patient care. Engage in clinical research activities related to mental health counseling, addiction counseling, dementia care, PTSD care, autism spectrum disorders, and other developmental disabilities. Maintain accurate documentation of patient interactions, progress notes, treatment plans, and medical records using electronic medical record (EMR) systems like ICD coding standards (ICD-9/10). Experience Valid licensure as a Licensed Clinical Social Worker (LCSW) or equivalent in the respective state or region. Proven experience working with diverse populations in settings such as hospitals, nursing homes, assisted living facilities, or outpatient clinics. Demonstrated expertise in child & family counseling, hospice care, geriatrics, pediatrics, or working with individuals with disabilities. Familiarity with medical terminology, CPT coding for billing purposes, ICD coding standards for diagnoses (ICD-9/10), and HIPAA compliance for patient privacy. Prior experience in crisis management, post-acute care environments, or home & community-based services is highly desirable. Strong skills in data collection for research purposes and proficiency with EMR systems to document clinical encounters accurately. $30.00 - $37.29 per hour
