Social Worker

St. John Family Medicine 8345 Wicker Ave St John, Indiana 46373 Patients and families' healthcare often need guidance and counseling to help resolve problems and deal with what might be a very different way of life post-illness. Our Social Workers are an important link in our home healthcare team. These are the people who educate staff and families and find needed support and resources. This position will provide support to our Family Medicine and Behavioral Health departments.

WHAT YOU CAN EXPECT

Schedule:

Monday - Friday, Day shift, 40 hours per week (no weekends or company recognized holidays) Collaborate with multidisciplinary teams to evaluate and assess patients' medical and physical needs. Refer patients to appropriate services if patients are identified with depression, anxiety, grief or mental illness. Utilize consultation data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure service efficacy. Identify environmental impediments to patient progress through interviews and review of patient records. Assist patients and their caregivers in identifying goals of care and assists with unmet needs through linkages to community resources. Assist in crisis intervention. Document patients' electronic medical records in a clear, concise, accurate, and timely manner.

QUALIFICATIONS

Bachelor's Degree in Social Services field required 1 year of Social Work experience in an ambulatory setting preferred Indiana Licensed Social Worker (LSW) required
