Job description Kaiser Permanente seeks a Medical Social Worker III to deliver advanced clinical social work services within our Healthcare & Medical Services department. In this role, you will perform comprehensive biopsychosocial assessments, coordinate complex discharge planning, and connect patients and families with critical community and behavioral health resources. You will collaborate closely with an interdisciplinary care team to support treatment adherence, patient safety, and continuity of care. The Medical Social Worker III provides crisis intervention, risk assessment, and advocacy while maintaining thorough documentation in the electronic health record and adhering to regulatory and ethical standards. This position offers the opportunity to contribute to quality improvement initiatives and mentor junior staff while supporting a patient-centered, culturally responsive care environment.
