Summary of Duties and Responsibilities:
Responsible for the delivery of social work services (BSW and MSW) including initial psychosocial assessments (MSW), ongoing direct social work needs, crisis intervention, bereavement services, family referrals and linkages, and community education and outreach.

Director of Social Services Job Details and Essential Functions:
Performs psychosocial assessments (MSW) on terminally ill patients with input from family Performs ongoing reassessments; provides education, resources, what to expect during the dying process in hospice, crisis intervention as appropriate; and utilizes supportive tools and referrals for patients/families as necessary or requested Documentation is completed in the patient's electronic medical record (EMR) within 24 hours of services Actively participates in coordination of all aspects of patient's care, in accordance with current professional standards and practice Participates/contributes in ongoing interdisciplinary team meetings Develops, assesses, and maintains the Plan of Care (POC) and patient documents in the EMR Serves as an advocate and point of support for the patient/family unit, assists with administrative, regulatory, and financial procedures/paperwork, including final arrangements Serves on-call duties and maintains caseload as assigned by Supervisor. Caseloads may vary based on management's evaluation of patient/family and Foothills needs Participates in patient care meetings, and other Hospice meetings as requested Participates in Quality Assurance meetings Participates in FCC orientation and in-service programs for staff and volunteers and program and Hospice sponsored in-services Performs other tasks/duties as assigned by the Director of Social Services May be assigned to any workplace setting based on needs including office, RCFE, SNF and Home Knowledge and Skills Required Ability to use independent judgement to make sound decisions Maintain confidentiality and follow HIPAA and patient privacy laws Strong computer and clerical skills such as knowledge and experience with Microsoft products, ability to use a keyboard for typing, familiarity with copying, scanning and faxing Critical thinking skills and independent thinking skills to select and deliver necessary social work and crisis interventions Communicate effectively, both orally and written Public speaking, developing relationships with identified partners, groups, businesses with whom Foothills Compassionate Care have or need to establish affiliation Education & Experience Required Bachelor's degree in social work Two years' experience in family and individual social work and crisis intervention or home health social work Desired Experience in a hospice and/or home health position Knowledge of state and Medicare regulations for service delivery and documentation License / Certifications Required Bachelor's degree in social work Current CPR certificate Valid California driver's license, a reliable automobile and evidence of appropriate liability insurance
Pay:
$60,000.00 - $82,000.00 per year
