About the Role As a Medical Social Worker, you will provide psychosocial support, counseling, and resource coordination for hospice patients and their families. This Role If You:

Provide compassionate support to patients and families navigating serious illness Communicate effectively with patients, families, and care teams Coordinate resources, referrals, and community support services Maintain professional boundaries while fostering trust and understanding Work independently in the field while collaborating with a supportive team This role is ideal for social workers who find meaning in compassion, advocacy, and connection — not just documentation. Qualifications Master's degree in Social Work (MSW) Required Current license in good standing (LCSW, LICSW, or equivalent) Experience in hospice, palliative care, or healthcare social work preferred Valid driver's license and reliable transportation Strong clinical judgment, communication, and organizational skills
