About the PHF Our 24-hour LPS-designated Psychiatric Health Facility in Oceanside provides short-term inpatient stabilization (average 5-7 days) for adults 18+ experiencing acute psychiatric or co-occurring conditions.

Rapid stabilization Collaborative treatment planning Meaningful discharge planning Strong community reintegration You'll work closely with psychiatry, nursing, case management, and support staff in a structured but dynamic environment where your clinical voice matters. What You'll Do As a Social Services Coordinator II, you will: Conduct comprehensive psychosocial assessments Provide therapeutic interventions and crisis support Lead discharge planning and community linkage efforts Collaborate with interdisciplinary team members daily Coordinate with community providers, housing resources, and support systems Advocate for clients during one of the most vulnerable moments in their lives You'll play a central role in helping patients stabilize and transition successfully back into the community. Qualifications Required:
Current, valid, and unrestricted California clinical license (LCSW, LMFT, PhD, PsyD) Master's or doctoral degree in a behavioral health field 3-5 years of clinical experience in mental health Strong crisis intervention and assessment skills Excellent written and verbal communication Ability to prioritize and manage time effectively
Preferred:
Experience in inpatient, crisis, or acute psychiatric settings Current CPR certification Why Work at Exodus?
