Licensed Clinical Social Worker

Job Summary Crown Community Health Center is seeking a qualified Licensed Clinical Social Worker (LCSW) to provide clinical social work services in a Community Mental Health Center / Partial Hospitalization Program setting. This is a part-time/per diem position requiring services once per week . The LCSW will work with adults experiencing depression, anxiety, psychosis, bipolar disorder, social isolation, functional decline, and difficulty managing daily life stressors. The LCSW will provide psychosocial assessment, treatment planning support, group intervention, individual support when needed, discharge planning support, and coordination with the multidisciplinary treatment team. Responsibilities Provide clinical social work services to adult

CMHC/PHP

clients. Complete psychosocial assessments and assist with treatment planning. Facilitate therapeutic groups focused on coping skills, emotional regulation, socialization, communication skills, community resources, relapse prevention, and independent functioning. Provide supportive counseling and crisis support when clinically appropriate. Assist clients with identifying barriers to treatment, social needs, family concerns, housing concerns, transportation issues, and community resource needs. Document client participation, progress, symptoms, barriers, and response to interventions. Collaborate with psychiatrist/PMHNP, nursing staff, occupational therapist, activity staff, and administration. Participate in multidisciplinary treatment team meetings as needed. Maintain professional, ethical, and client-centered care. Requirements Current and valid California LCSW license required. Ability to work with adults experiencing serious mental health symptoms. Strong clinical documentation and communication skills. Ability to facilitate therapeutic groups. Ability to work as part of a multidisciplinary treatment team. CPR/BLS certification preferred. Preferred Qualifications Experience with adults or older adults in a structured mental health program. Knowledge of psychosocial rehabilitation, coping skills training, care coordination, and discharge planning. Work Environment This position is located in a structured outpatient mental health program. Services are provided during daytime program hours, once per week.

Pay:

$45.00 - $55.00 per hour
