  • 68/hr (Depending on Experience) Job Summary "Navitas Healthcare, LLC" is seeking an experienced Medical Social Worker (MSW) for an acute care hospital setting in Torrance, California.
The MSW will be responsible for discharge planning, psychosocial assessments, care coordination, and resource management for hospitalized patients. This role requires strong hospital-based case management experience and the ability to manage complex patient discharge needs in collaboration with interdisciplinary teams. Responsibilities Conduct psychosocial assessments for hospitalized patients Develop and implement discharge plans for safe patient transitions Coordinate post-discharge services and community resources Collaborate with physicians, nurses, case managers, and interdisciplinary teams Provide emotional, social, and financial support to patients and families Arrange placements for rehabilitation, skilled nursing, home health, and community services Provide crisis intervention and supportive counseling as needed Document assessments, interventions, and discharge planning accurately Monitor patient progress and address barriers to discharge Participate in multidisciplinary rounds and care planning meetings Cover assigned units and assist with weekend discharge planning Ensure compliance with hospital policies and patient confidentiality standards Required Qualifications Master's Degree in Social Work (MSW) from an accredited program Minimum 2 years of recent acute care hospital case management experience Strong experience in hospital discharge planning and care coordination Ability to manage multiple complex cases in a fast-paced environment Availability for every other weekend and holiday coverage Strong communication and interdisciplinary collaboration skills For more details contact at or Call / Text at 516-862-1169.
