  • Full-time, Contract Overview Nurse Next Door is seeking a compassionate and experienced Medical Social Worker (MSW) to provide home health services to patients throughout the Sacramento region.
The Medical Social Worker will assess patients' psychosocial needs, provide counseling and community resource coordination, and assist patients and families in navigating social, emotional, financial, and environmental challenges related to illness and recovery. This is a 1099 independent contractor position offering flexible scheduling and competitive per-visit compensation. Essential Duties and Responsibilities Conduct psychosocial assessments in patient homes Identify emotional, social, financial, environmental, and community support needs Develop individualized care plans in collaboration with the interdisciplinary team Provide counseling and emotional support to patients and families Assist patients with access to community resources and support services, including: Transportation assistance Housing resources Financial assistance programs Advance care planning Long-term care planning Caregiver support services Coordinate referrals to community agencies and social service programs Assist with discharge planning and transition of care needs Educate patients and families regarding available healthcare and social service resources Participate in interdisciplinary care conferences Complete documentation accurately and within required timelines Maintain compliance with Medicare, HIPAA, California Department of Public Health (CDPH), and agency regulations Qualifications Master's degree in Social Work (MSW) from an accredited program Current California Social Work license preferred (LCSW preferred but not required depending on role) Minimum one (1) year of healthcare or medical social work experience preferred Home health or hospice experience preferred Knowledge of community resources within Sacramento and surrounding counties Current CPR/BLS certification preferred Valid California driver's license Reliable transportation and proof of auto insurance Strong communication, counseling, and organizational skills Ability to work independently and manage patient caseload effectively Independent Contractor Requirements Maintain active professional liability/malpractice insurance Responsible for all taxes and independent contractor obligations Maintain all required licenses and certifications Provide own transportation, phone, and basic work equipment Compensation Competitive per-visit compensation based on visit type and experience: Visit TypeCompensation Range Initial MSW Evaluation$100
