Institute on Aging | On Lok PACE (100% onsite) Are you a compassionate Medical Social Worker who thrives in a collaborative, mission-driven healthcare environment? Join On Lok PACE at the Institute on Aging (IOA) and play a critical role in supporting frail elders to live safely, independently, and with dignity in their communities. About the Role The Medical Social Worker provides comprehensive psychosocial assessment, counseling, and care coordination to participants with complex medical and social needs. You'll serve as a key liaison between participants, families, and the interdisciplinary care team, ensuring care plans are participant-centered, holistic, and responsive. What You'll Do Partner closely with Primary Care Providers, Registered Nurses, Care Managers, and other disciplines Provide supportive counseling, crisis intervention, advocacy, and therapeutic interventions Facilitate family and care team meetings, including end-of-life planning and goals of care Coordinate care for participants with complex needs, including transitions and discharge planning Connect participants and families to community resources and behavioral health services Advocate for participants around mental health, substance use, housing instability, and chronic psychosocial needs Maintain accurate and timely documentation in the electronic health record (EHR) Participate in quality improvement initiatives, IDT meetings, and program activities Stay current with professional standards, continuing education, and ethical practices
What We're Looking For Required:
Master's degree in Social Work (MSW) from an accredited program Minimum 1 year of experience working with frail elderly populations Minimum 1 year of experience in an interdisciplinary care model
Preferred:
